When you go trekking at altitude, it makes sense to have insurance to cover the cost of the unlikely event that you actually need it. Most standard travel insurance policies will only cover trekking up to 2500/3000 meters, unless you buy a winter sports or add on option to cover you. To go trekking in Nepal for example you are required to have helicopter rescue insurance, it's a requirement not only to save you the cost of paying; but you need it to get a park entry permit for both Everest and Annapurna.
How To Get A Good Deal
National mountain clubs offer very good insurance at a lower cost than commercial insurance agents
Examples.
AAC American Alpine Club
BMS British Mountaineering Club
Austrian Alpine Club
OTHER OPTIONS
The other option is a rescue insurance package from Global Rescue. You only need to buy this type of insurance for the actual time you need it, for example you are planning a 14 day holiday and for 5 days you will be in the mountains, in this case you just buy for the time you need.
EXCESS
Some polices have a very high excess so this is something to think about before you buy
